By submitting this form, you acknowledge and agree to follow the payment schedule outlined in our payment calendar. If you attend any classes during a trimester, you are responsible for the full tuition associated with that trimester.

By submitting this form, you also agree to release Limelite Dance Studio, LLC and its instructors, employees, and representatives from any liability for injuries, accidents, or damages that may occur during classes, on or around the studio premises, or at studio-sponsored events.

Limelite Dance Studio, LLC may photograph or record classes, performances, and events for promotional, educational, and social media purposes. By submitting this form, you grant permission for the use of such images and recordings unless written notice is provided to the studio.

In the event of an emergency, I authorize Limelite Dance Studio, LLC to seek appropriate medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any resulting medical expenses.

I understand that participation in dance and physical activity involves inherent risks of injury. I voluntarily assume all risks associated with participation in classes, rehearsals, performances, and studio events

I agree to keep all contact and emergency information current and to promptly notify the studio of any changes.

I acknowledge that I have read and agree to abide by all studio policies, including those related to tuition, attendance, costumes, recital participation, and student conduct.
